You must apply for service retirement benefits before your PEERS retirement date by filing the Service Retirement Application and other required forms listed below. You need the free Adobe® Reader® to view these forms.

Service Retirement Application Packet (Includes all the forms listed below)
This electronic packet contains all the forms you need to apply for service retirement. View and print them all at once by opening this file. If you choose PLSO, you also need the PLSO Distribution Form.
Service Retirement Application
Your retirement application tells us when you want to retire, which benefit plan you choose for the payment of your benefits, and who your post-retirement beneficiaries are.
Tax Withholding Authorization for Monthly Benefits
This form tells us the federal and state income tax amounts to withhold from your benefits.
Direct Deposit Authorization for Monthly Benefit Payments
Use this form to provide us bank account information for the direct deposit of your benefits.
PLSO Distribution Form (File only if you choose the PLSO)
This form tells us which PLSO payment you choose, and gives both PEERS and your financial institution direction if you want to rollover your PLSO payment to another qualified retirement account.
