Critical Shortage Enrollment Request Form

To certify a position as qualified for Critical Shortage Employment, please complete and submit the following form.

Once we have reviewed and approved the completed form PSRS/PEERS will mail a certification form to the superintendent. The form must be completed, signed and returned to PSRS/PEERS.

Is the employment full-time or part-time?

Employer Staffing Information

Has the Employer Done the Following:

Position deemed to be a critical shortage position for the current school year.

Offered a retirement incentive for either of the previous two school years?

Posted the vacancy for at least one month.

Solicited applications through local newspaper, other media or teacher education programs.

Made a good faith effort to fill the position with non-retired employees.

Determined that there is an insufficient number of eligible applicants for the advertised position.
