Verification of Date of Birth
Why does PSRS need verification of my date of birth?
We must verify the dates of birth of 1.) all retiring members 2.) beneficiaries under Option 2, 3 or 4, and 3.) survivors of deceased members eligible to receive benefits. Verification is needed to determine eligibility and for actuarial calculations.
What is required?
A photocopy of the birth certificate bearing the original seal of the city, county or state in which it was issued. Hospital-issued birth certificates are not accepted. Original birth certificates are returned to you.
What if I do not have a birth certificate?
If you were born in Missouri, click here for information on obtaining a birth certificate from the Bureau of Vital Records.
If you were born in another state, click here for information on obtaining a birth certificate from the state in which you were born.
What if I cannot obtain a birth certificate?
You may submit three documents that show your date of birth or your age as of a certain date. At least one must show your date of birth. Original documents will be returned.
Documents which might be acceptable in lieu of a birth certificate include:
